MEGA MATH, LLC
POLICY STATEMENT
Please read the MEGA MATH, LLC policy statement below. To ensure that the MEGA MATH program runs effectively and serves its purpose of providing a fun and enriching math learning experience for all enrolled students, it is important that parents, students, and any other caretakers understand and adhere to the policies of MEGA MATH, LLC.
CLASSES. All classes are 90 minutes in length unless otherwise arranged. Classes start on time and consist of a combination of instructor-led conceptual learning, worksheet completion, activities, games, logic puzzles, and peer support. The class instructor guides student learning and activities and works to ensure deep understanding of math concepts. Each class will have one instructor, though a substitute instructor may teach the class if the primary instructor is sick or unable to teach for any other reason. Parents will be notified if a substitute instructor is going to teach a class.
CLASS PLACEMENTS. In general, students will be placed with same-age and/or same-grade peers. In some instances, students may be placed in a class that does not correspond to their grade in school. This decision will be made by MEGA MATH instructors based on assessments, observations, and any other pertinent information about the child’s math skills and math learning journey.
CLASS MATERIALS. There is no charge for class materials; however, if a student damages or loses any materials or supplies, parents will be asked to replace them unless otherwise arranged.
ENROLLMENT. Enrollment is limited for each class. MEGA MATH, LLC reserves the right to increase or decrease class enrollments. Parents will be notified of any increases in enrollment should it be greater than the number of students originally stipulated. Enrollment increases will only take place if it maintains or benefits the learning environment for any particular class. The annual registration fee is $160 and is non-refundable. MEGA MATH, LLC requires at least 30 days’ notice for any class changes or withdrawal from the program, unless otherwise arranged.
BEHAVIORAL EXPECTATIONS AND DISCIPLINE. Children enrolled in the MEGA MATH, LLC program are expected to follow the rules of the program. They are expected to be respectful of their peers and to their instructor and to be courteous and participatory in class learning activities. Classes are held at our office at 213 E Street, Suite F. MEGA MATH classes are intended to be safe spaces for all children in the class. Repetitive disruptive or disrespectful behavior could result in dismissal from the class. There is zero tolerance for bullying in MEGA MATH classes. Deliberately mean, hurtful, or emotionally injurious language or behavior is likely to lead to a student’s dismissal from the program.
ATTENDANCE. Attendance at weekly classes is not required, though students are encouraged to attend as much as they can to get the full benefits of the MEGA MATH program. Parents are kindly asked to inform class instructors of an expected absence.
TRANSPORTATION. Parents and caretakers will be responsible for transporting students to and from class. Late pick-ups will be assessed at a charge of $1.00 per minute after five minutes from the time class has ended.
COMMUNICATION. Parents are asked to add info@megamathdavis.com to their email contact list to ensure that email communications do not go to email spam folders. Parents are encouraged to review their child’s math work from their MEGA MATH binder from time to time to stay up to date on their child’s progress. Parents ought to communicate any questions or concerns that arise to the class instructor. Phone calls and emails are welcome and will be returned in a timely manner. There is no charge for calls under 10 minutes duration. Parents may request conferences with instructors at a rate of $50 for a 30-minute conference.
PAYMENTS. Parents will be invoiced at the end of the month and payments are due by the 15th of each month unless otherwise stipulated. Late payments will be assessed a fee of $10/day if not received by the due date on the invoice, unless otherwise arranged.
MEGA MATH, LLC POLICY AGREEMENT AND WAIVER
In consideration for allowing the student named above to participate in the MEGA MATH, LLC educational program (hereinafter referred to as the "Program"), the undersigned parent/guardian (hereinafter referred to as "Parent") hereby acknowledges and agrees to the terms and conditions set forth below:
1. Policy Agreement: Parent has read and understands the above MEGA MATH, LLC Policy Statement. Parent understands the enrollment, class placement, and payment policies along with behavioral expectations of the student.
2. Governing Law and Jurisdiction: This agreement shall be governed by and construed in accordance with the laws of California, without regard to its conflicts of laws principles. Any legal action arising out of or in connection with this agreement shall be filed in the appropriate court of Davis, located in Yolo County, California.
3. Medical Attention: Parent authorizes the MEGA MATH, LLC’s staff, associated parties, or agents to seek medical treatment for the student in case of an emergency. Parent certifies that the student is in good health and is physically and mentally capable of participating in the MEGA MATH, LLC program activities.
4. Assumption of Risk: Parent acknowledges that participation in educational activities, including those offered in the MEGA MATH, LLC Program, may involve certain inherent risks. These risks include but are not limited to physical injuries, emotional distress, and exposure to new ideas and concepts. Parent understands and voluntarily assumes these risks on behalf of the student.
5. Release and Waiver: Parent, on behalf of themselves and their heirs, executors, administrators, and assigns, hereby releases, discharges, and holds harmless the organizers, sponsors, instructors, staff members, and associated parties of the MEGA MATH, LLC program from any and all claims, liabilities, injuries, losses, or damages arising out of or in connection with the student's participation in the Program.
6. Indemnification: Parent agrees to indemnify and hold harmless the organizers, sponsors, instructors, staff members, and associated parties of the MEGA MATH, LLC program from any claims, actions, damages, liabilities, costs, or expenses arising out of or in connection with any breach of the Parent's representations, warranties, or obligations under this agreement.
9. Severability: If any provision of this agreement is found to be invalid, illegal, or unenforceable, the remaining provisions shall continue to be valid and enforceable to the fullest extent permitted by law.
